上網填寫申請表，於三個工作天後至選擇的受理縣市驗證&繳費領件

　# 臨櫃辦理
至警察局外事科填寫申請表，3~7個工作天後自領或郵寄領取

如果需要委託其他人辦理或領取，受委託人需攜帶委託書

說明：

核發證明書需三個工作天，但因涉案未結需向司法或軍法機關查詢者不在此限。
It will take three working days for issuing this certificate . Sometimes, waiting period will be longer because court records need to be checked.
領證及繳費事項請洽受理之縣市警察局，非本人申請需另外檢附委託書正本。
Information on pick-up-records and payment of processing fee, please check with the police agency in charge; If application made by proxies, a letter of authorization must be submitted (original only).Personal ID of proxies is required.
網路申請者，於上網申請後第3日起30日內攜帶應備證件至受理單位外事課櫃台驗證及繳費領件，逾期且未繳費之申請案件作廢，請重新申辦。
To apply online, please pick up your record and pay the processing fee at the foreign affairs section service counter of the police agency in charge within the next 30 days from the third day of application. Applicant’s and proxy’s (if any) ID are submitted to verification while the record is being picked up. The application will be declared void if the applicant fails to pick up the record within the duration of issuing.
